The qSOFA (quick Sequential Organ Failure Assessment) score is a simplified screening tool developed to help clinicians outside the ICU quickly identify patients at high risk of poor outcomes due to infection. While it is not a diagnostic tool for sepsis itself, a score of 2 or more indicates a significantly higher risk of mortality or extended ICU stay.

The tool checks the patient's vitals and mental status against three specific thresholds. For every threshold met (RR ≥ 22, SBP ≤ 100, or GCS < 15), one point is added to the score.
