qSOFA Score Calculator

Quickly assess a patient's risk for sepsis with the quick Sequential Organ Failure Assessment (qSOFA) score.

Sepsis is a medical emergency that requires rapid recognition and treatment. The qSOFA (quick Sequential Organ Failure Assessment) score is a simplified screening tool developed to help clinicians outside the ICU quickly identify patients at high risk of poor outcomes due to infection. While it is not a diagnostic tool for sepsis itself, a score of 2 or more indicates a significantly higher risk of mortality or extended ICU stay.

Educational use only. This tool does not replace clinical judgment or local protocols.

Result

0 / 3

qSOFA Score: 0

Respiratory rate (/min)18 /min
Systolic BP (mmHg)110 mmHg
Altered mentationNo

qSOFA uses RR ≥ 22, SBP ≤ 100, altered mentation.

Nasıl Çalışır

The tool checks the patient's vitals and mental status against three specific thresholds. For every threshold met (RR ≥ 22, SBP ≤ 100, or GCS < 15), one point is added to the score.

Hesaplama Formülü

1 point for each: RR ≥ 22, SBP ≤ 100, GCS < 15

Calculation Examples

1. **High Risk**: A patient with an infection has a BP of 90/60 (1 pt) and is breathing 24 times a minute (1 pt). qSOFA = **2**.

Why use this tool in your daily life?

Time is tissue. This service provides a standardized, evidence-based method for sepsis triage, empowering floor nurses and ER staff to advocate for higher levels of care for at-risk patients.
